An Israeli Defense Forces (IDF) investigation has determined that Ofer Mushkovitz, who was killed in the community of Misgav Am, was struck by artillery fire that did not comply with standard operational procedures. The Northern Command's commander, Rafi Miloh, indicated that there were multiple severe errors in planning and executing the shelling during the incident. The investigation found that five shells hit Misgav Am instead of the intended enemy targets due to incorrect firing angles and failures in adhering to necessary regulations. He mentioned that lessons would be learned from the investigation's findings.

Initially, it was reported that Mushkovitz, 60, died due to a strike from Hezbollah; however, the IDF later clarified that they were assessing whether the artillery fire was errant due to soldiersโ€™ actions leading to damage to his vehicle. Mushkovitz was known for his work in agriculture, managing avocado orchards, and had previously served as the spokesperson for his kibbutz and chairman of its management committee. Mushkovitz was visiting his daughter in Pardes Hanna for his grandson's circumcision, and shortly after returning to Misgav Am, the tragic event occurred. He leaves behind three daughters and grandchildren, one of whom was born just two weeks prior. His death has prompted discussions about military protocols and the need for improved operational measures to prevent such incidents in the future.
