Health plans record a profit of R$ 24.4 billion in 2025, with adjustments above costs

Health plans in Brazil reported a record profit in 2025, driven by higher monthly fee adjustments and strong financial results despite high interest rates.

In 2025, Brazil's health plans registered a historic profit of R$ 24.4 billion, marking the highest figure in the industry's history amid substantial adjustments to monthly fees that outpaced assistance costs. According to data released by the National Health Agency (ANS), the sector generated revenues of R$ 391.6 billion, translating to a profit margin of approximately 6.2%. This indicates that for every R$ 100 earned in revenue, the health plans made about R$ 6.20 in profit, which is higher than the profits recorded during the COVID-19 pandemic.

The financial figures reveal significant concentration within the sector, as three of the largest operators accounted for nearly 49% of the total profits, suggesting a monopolistic tendency in the health plan market. Despite these large players dominating, the overall performance of health plans has seen an improvement, exemplified by a rise in the number of operators available in the market. This development indicates a competitive edge that could benefit consumers, alongside a reported return on equity (ROE) of 16.4%, surpassing pre-pandemic levels.

These record profits and the structural dynamics within the health plan sector raise important questions regarding affordability and access to healthcare for everyday Brazilians. As the industry continues to demonstrate robust financial health, the challenge will be to ensure these profits translate into fair premiums and quality care for consumers. The situation warrants close monitoring by regulatory agencies to protect the interests of the users amid an industry where the balance of power increasingly favors a few large corporations.
