A reinvestigation committee found 12 instances of bullying contributing to the suicide of a second-year student at Tokai University Fukuoka High School, highlighting failures in the school's crisis management system.

A second-year male student at Tokai University Fukuoka High School in Munakata City, Fukuoka Prefecture, took his own life in 2021, prompting a reinvestigation by the Prefectural Committee which reported on April 13 that there were 12 instances of bullying, including sexual harassment by seniors. The committee, led by lawyer Koji Ito, concluded that while the bullying was a serious issue, it could not directly attribute the suicide to these instances alone. They pointed out systemic issues in the school's crisis management approach, which failed to effectively address or report incidents of bullying.

The report details that the student experienced horrific abuse, including being restrained with tape in the clubroom and being filmed in compromising situations, which were then shared on social media. Other bullying actions included confiscating the student's smartphone, sending inappropriate messages to girls using his device, and physical assaults. Approximately one year and nine months after these acts of bullying began, the student tragically took his own life, illustrating the long-term psychological impact bullying can have.

The committee warned that the school's failure to implement proper reporting and management protocols for bullying allowed the situation to escalate. They indicated that a lack of understanding of bullying within the school environment contributed to its normalization and deepening. Furthermore, they acknowledged that the student's suicide was influenced by a combination of complex personal factors, including a difficult family environment, loss of trust with the club advisor, and low self-esteem, which collectively led to the tragic outcome. This case raises profound concerns about the effectiveness of school policies on bullying prevention and psychological support for students.
