The collision took place along the Okota–Mile 2 route when a yellow commercial minibus, known locally as Faka, was forced to perform sudden braking due to an unknown private vehicle reversing onto the carriageway. The vehicle, traveling at high speed, lost stability during this abrupt maneuver, resulting in a rollover.

According to Adebayo Taofiq, the Director of the Public Affairs and Enlightenment Department of the Lagos State Traffic Management Authority, preliminary investigations indicated that the private vehicle's reckless action directly caused the catastrophic incident.
